The Social Work Practitioner – Program Manager (SWP-PM) will work within a multidisciplinary team to provide direct social work services to include client assessment, education, and coordination of resources to keep clients engaged in health care and treatment adherence.
The SWP is expected to identify and address complex barriers that prevent adherence to prescribed treatment plans; develop short and long-term care plants, determine new treatment approaches, collaborate, and coordinate with other service delivery systems, and meet the needs of the client receiving treatment at ECUHIVP.
SWP is expected to coordinate client referral to a host of core HIV medical services: primary HIV medical care, specialty care, mental health, and substance abuse treatment, oral health, HIV medications/treatment adherence counseling, health and risk reduction education & related services, early intervention services, home health care, medical nutrition therapy, hospice services, community-based health services, clinical quality care measures, HIV education and advocacy; and participating in the expansion of social work services at ECUHIVP.
SWP-PM management responsibilities include ensuring the patient access staff and case management team work collaboratively to schedule client visits appropriately, collect RWE documents, ensure RWE clients get financial counseling services to meet RWE requirements.
Due to the nature of the job, the work schedule is flexible, including possible nights, and weekends.

License or Certification required by statute or regulation None Preferred Experience, Skills, Training/Education Bachelor’s Degree in social work or Masters Degree in social work from an accredited school of social work and three years experience to include case management, client assessments and referrals, and psycho-social support and counseling.
Experience with HIV+ clients and clients with other infectious diseases are highly desirable.
Working knowledge of the Ryan White program is preferred.
